{"id":3269,"date":"2015-06-15T15:00:00","date_gmt":"2015-06-15T15:00:00","guid":{"rendered":"http:\/\/www.emlitofnote.com\/?p=3269"},"modified":"2015-06-15T15:00:00","modified_gmt":"2015-06-15T15:00:00","slug":"chillin-children-after-ohca","status":"publish","type":"post","link":"https:\/\/www.emlitofnote.com\/?p=3269","title":{"rendered":"Chillin\u2019 Children After OHCA"},"content":{"rendered":"<p>Once upon a time, many adults suffering an out-of-hospital cardiac arrest received therapeutic hypothermia with a target temperature of 33\u00b0C. &nbsp;Then, along came the <a href=\"https:\/\/www.emlitofnote.com\/2013\/11\/giving-hypothermia-cold-shoulder.html\">Targeted Temeperature Managment trial<\/a> \u2013 in which 36\u00b0C seemed to be just as good as 33\u00b0C. &nbsp;Now, just to throw another confounder in the mix, we have a trial comparing 33\u00b0C to \u201ctherapeutic normothermia\u201d \u2013 36.8\u00b0C \u2013 and we\u2019re doing it in children to address concerns regarding generalizability from adults.<\/p>\n<p>Very detailed summaries of the numbers, methods, and enrollment can be found on other <a href=\"https:\/\/twitter.com\/FOAM_Highlights\">#FOAMed<\/a> sites \u2013 particularly <a href=\"http:\/\/stemlynsblog.org\/paed-therapeutic-hypothermia\/\">St. Emlyns<\/a> and <a href=\"http:\/\/www.aliem.com\/therapeutic-hypothermia-after-pediatric-cardiac-arrest-out-of-hospital-thapca-oh-trial\/\">ALiEM<\/a>. &nbsp;But, the high points:<\/p>\n<ul>\n<li>Many \u2013 1,355 \u2013 were screened, but ultimately only 260 were randomized and included in their primary analysis.<\/li>\n<li>Adherence to temperature management protocols was good or adequate in ~90% of cases.<\/li>\n<li>Hypothermia was implemented for 48 hours, followed by normothermia up to 120 hours total to match the normothermia group.<\/li>\n<li>In contrast to adults, the great majority (72%) of this pediatric cohort suffered a respiratory arrest.<\/li>\n<\/ul>\n<p>The outcome: no statistical difference, with 20% of the hypothermia group alive and functional at 1 year, compared with 12% of the normothermia group, a p-value of 0.14. &nbsp;Regarding safety, arrhythmias and culture-proven infections favored the normothermia group, 1% vs. 5%, and 39% vs. 46%, but these also did not reach statistical significance. &nbsp;Finally, both 28-day and 1-year mortality favored hypothermia, with an absolute difference of ~10% in each, but this was not statistically significant, either.<\/p>\n<p>I will let the authors speak for me here:<\/p>\n<blockquote><p>\u201cOne important potential limitation of the trial is that, on the basis of the observed confidence limits for treatment differences, a potentially important clinical benefit cannot be ruled out despite the lack of a significant difference in the primary outcome measure. A larger trial might have detected or rejected a smaller intervention effect. Indeed, there was a significant difference in survival time with therapeutic hypothermia, although this was a secondary outcome measure.\u201d<\/p><\/blockquote>\n<p>The relative likelihood of benefit for hypothermia in this trial was 1.54, with a 95% CI of 0.86 to 2.76. &nbsp;Now, this result crosses 1, and therefore requires interpretation in two contexts. &nbsp;The first is the normal distribution:<\/p>\n<div style=\"clear: both; text-align: center;\"><a href=\"http:\/\/upload.wikimedia.org\/wikipedia\/commons\/a\/a9\/Empirical_Rule.PNG\" style=\"margin-left: 1em; margin-right: 1em;\"><img decoding=\"async\" loading=\"lazy\" border=\"0\" src=\"http:\/\/upload.wikimedia.org\/wikipedia\/commons\/a\/a9\/Empirical_Rule.PNG\" height=\"290\" width=\"400\" \/><\/a><\/div>\n<p>In which we visualize the frequency of potential outcomes, and the important realization the more frequent &#8220;true&#8221; outcome is most likely to occur near the center of the 95% CI range.<\/p>\n<p>And, the more important context:<\/p>\n<div style=\"clear: both; text-align: center;\"><img data-recalc-dims=\"1\" decoding=\"async\" loading=\"lazy\" border=\"0\" height=\"226\" src=\"https:\/\/i0.wp.com\/4.bp.blogspot.com\/-v_BQYF6GfyA\/VXx_vgSpKQI\/AAAAAAAAAkQ\/Q2E-f3HYWJw\/s400\/p-graphic.jpg?resize=400%2C226\" width=\"400\" \/><span><\/span><span><\/span><a href=\"https:\/\/www.blogger.com\/\"><\/a><\/div>\n<p>In which we <a href=\"http:\/\/www.nature.com\/news\/scientific-method-statistical-errors-1.14700\">interpret these data in the context of prior results<\/a>, generalized from other settings. &nbsp;In this case, our prevailing opinion is one in which we suspect hypothermia \u2013 with much uncertainty regarding the details \u2013 is beneficial. &nbsp;As you can see, the effects of even \u201cstatistically significant\u201d findings have only limited practical impact on the &#8220;good bet&#8221;
